Located on the fourth floor, the Tŷ Hyder Reablement Unit provides support for up to twenty residents. The English translation of Tŷ Hyder is House of Confidence. Named to reinforce its objective, the Tŷ Hyder unit is an empowering place where confidence is grown and goals are achieved.
The purpose-built unit is a safe and well-equipped environment for individuals to achieve their desired goals away from the hospital setting, before returning home. The unit’s ethos is confidence and self-belief. Residents are encouraged and supported in the undertaking of basic self-care and other tasks. The Unit is an ideal environment for individuals to continue their rehabilitation and gradually increase their independence and confidence, enabling a safe transition from supported living to home. Comprising of an experienced group of staff , the Reablement team are dedicated to supporting residents on their reablement journey. The service is over seen by the Reablement Manager and the Reablement Team work in collaboration with local healthcare professionals including Physiotherapists and Occupational Therapists.
The newly refurbished facility is self-contained within Glain House and is accessed via a restricted access control door. Service users reside in single occupancy bedrooms equipped with their own television and access to Wi-Fi. The unit benefits from an open area for socialising, a lounge with kitchen and a laundry room, all for resident use. All residents are supported and encouraged to make their own drinks and snacks, launder their own clothing and bathe themselves, in addition to other daily tasks.
“The residential reablement unit in Glain House provides an environment in which individuals can continue to work towards greater independence ahead of returning home, including building confidence and practicing the skills that they need to live safely at home.
Shortly after arrival on the unit, they will have an assessment from a member of the therapies team who will help them to identify what they can already do for themselves, what they are currently finding more difficult and what their individual goals are. This will help us to put together a bespoke rehabilitation programme.
Throughout their stay on the unit, they will have regular input from the therapies staff and support from the care staff to work towards these goals. The therapies team will also work with them to put together a discharge plan for their return home, including referring on to any other organisations for ongoing support where needed after their stay.”
Glain House is situated on Newport Road in central Cardiff. The home benefits from excellent transport links and is a short walk away from the city’s train station and is easily accessible by bus .
